The information under each form is intended to assist you is finding the proper form and then completing it with the correct required information. All forms when completed, must  be sent to the District Administrator.

         Residency                Residence outside of chartered area. Applies to regular season only.

Inter-League Play/Combined Teams Applications
        Combined Teams  Used to draft teams from a common pool involving more than one league/division.
        Inter-League    Used when no combined tournament teams are being requested
        Inter-League and Combined Tournament Teams Check second box indicating level of team
                    (i.e. 9/10 Baseball) for which request is being made. Complete age break down for
                    involved teams.

Special Games Applications (Regulation IX)
        Special Games     For Little League chartered teams only.
        Special Games with non-LL teams    Used when non-Little League teams are involved.
